A copy of a rudimentary instructor input form is provided here. You may complete the form to provide information about the course you wish for a peer to review. Although its title includes the word “online,” you may adapt it for face-to-face instruction as well.
The following link will take you to a document entitled, “A Peer Review Guide for Online Courses at Penn State.” It is a very thorough qualitative rubric provided for Penn State’s peer instructional reviewers.
